MINIMUM STATE VACCINE REQUIREMENTS FOR TEXAS CHILDREN ENTERING KINDERGARTEN:
1. Three doses polio vaccine, including one received on or after
the 4th birthday

2. Four doses DTP/DTaP vaccine, including one received on or
after the 4th birthday
after the 4th birthday
3. Three doses hepatitis B vaccine
4. Two doses varicella vaccine received on or after 1st birthday
or written affidavit that the student has had the chickenpox
disease
5. Two doses MMR vaccine
6. Two doses hepatitis A vaccine or serologic confirmation of
immunity
or written affidavit that the student has had the chickenpox
disease
5. Two doses MMR vaccine

immunity
